Enliven Restorative Home Support is a service of Presbyterian Support East Coast, and is contracted to provide services in the Hastings and Napier cities and suburbs.  Each older person has a support programme based around their goals and aspirations.  The focus is on clients' abilities, not their limitations, with a view to keeping them in charge of their own destiny.

Support packages are driven by what is important to the client (their goals).  There are significant gains to be made in maintaining or restoring functional ability for older people if the support is oriented around what is important to the older person and translated into a support package with a level of flexibility.  It generally involves saturating a client with services (including socialisation) up front and building on their strengths until they are restored to a point where maintenance is the goal.  So the support package shrinks in size as the client progresses and gains independence.  Conversely it can be quickly increased if needed.

Underpinning the flexibility of the support package is the regular reassessment of the client by a clinically competetent RN case manager using evidence based screening and assessment tools as a part of a regular systematic structured review process.

Other key features are:

  • Being Strengths Based - working with what the client can do rather than focusing on what they cant do
  • A goal based approach - driven by what the client wants to achieve, no matter how unlikely.  It is about the process of supporting them to achieve their goals and the motivation that gives them to actively participate in gaining and maintaining their independence.  This process creates equality between the cleint and the service, it is a working partnership
  • A flexible and responsive approach where services are individually tailored to a support package
  • A functional activity of daily living programme built into the support package to restore strength and to extend the range of activities in which the client is independent
  • Reconnecting clients with their community and social activities
  • Enhanced Support Worker Training and Supervision
